Neo madness refers to an era after the "death" of the Madness Combat community which consists of people having a surface level interest of Madness and making it like every other fandom. The creator of this attribute the definition to current memes, shipping, and furrifying characters.
This definition has been watered down in multiple ways, the most notable being it attributing it to the spiritual death of madness, despite the named "death" being described before the neo madness era.
Person 1: "I saw a Sanford and Deimos meme where gay kiss"

The replacement of "mad cool". apart, these words do not mean much but together, they define awesomeness. This phrase can be used in just about any situation and depending on the tone used can mean anything you want it to.
