A man trips and trips someone else thus resulting in them falling into a puddle of mud. Someone shouts "Universal Fail".
When a person who is jokingly referred to as "immortal" in memes due to their great longevity and surprisingly good aging actually dies, causing mass hysteria and confusion.
When an immortality fail occurs, it spreads quicker than any other celebrity death. Typically, it takes less than an hour before everyone has heard the news.
The most recent immortality fail as of this post being written is Elizabeth II. With her death, Keith Richards is now the King of Immortals.
