Whereas 'mad as a box of frogs' clearly isn't that mad as a box is rigid and therefore easy to carry, a BAG of frogs would be quite insane to transport because of all that nutty hopping and ribbiting, it would slip out of your hands in quite an nutty manner.
Person a: See that Glyn?
Person b: yeah, I know him, he's mad as a bag of frogs.
Person a + everyone else: yeah, I agree. Quite barking mad.

Asking someone if there mad. Usally used after someone posts a serious thread about something. Pioneered in the teen misc section on body building forums.com. See example.
OP: today I broke my foot and now I can't squat for a month. Additionally someone stole my cellphone and ran up the bill.
Poster: u mad brah?!

When someone does a song cover that is infinitely better than the original. Comes from the cover of Tears for Fears "Mad World" by Michael Andrews and Gary Jules. Anyone who's seen the film Donnie Darko can testify it's awesomeness.
Jay Brannan's cover of The Verve Pipe's "The Freshmen" is another good example of Mad World Syndrome.

1) Intense misery and suffering, and distress.
2) Insufferable static cling, pinching and bunching (and embarrassment?) wearing this dress.
EXAMPLE #1:
"Raul's natural insecurity as an illegal alien and his angst in social settings caused him MAD AGONY IN 'DISDRESS' as he suffered through the long chow line at Hobo Park."
EXAMPLE #2:
"I gotz me MAD AGONY IN 'DISDRESS' because I have some unresolved issues. Maybe you'll like it if it fits you, Raul. Or should I call you Raula?"."
