The actual exchange rate between (say) the yen and the US dollar tells you nothing about the relative strength of the two currencies. The US dollar buys 92.57 yen (17 May 2010) right now, which sounds like a lot. But $100, converted into ¥9,257, only buys $71 worth of actual goods & services. In order for the yen:dollar exchange rate to reflect real purchasing power of the two currencies, the US dollar should be able to buy ¥130.
The real exchange rate for the US dollar against the Japanese yen is 1.41 (meaning the yen is costlier than the dollar in real terms).
The recent (March-May) fall of the euro against the US dollar has brought the real exchange rate of the two currencies into approximate parity.

Bills of exchange were adopted in 13th century Italy; almost as soon as they became common, traders began to use them as a speculative instrument (discounting bad ones and reselling them) or else as a sleazy method of borrowing money (by "drawing and redrawing," i.e., where two merchants in different towns agree to exchange bills of exchange with each other). "Drawing and redrawing" is analogous to the method used by college students on the 1980's of writing checks to each other every couple of days and depositing them in ATM's so their checking accounts wouldn't bounce.

When someone doesn't follow the international exchange rate and takes you for a ride. Usually common among people who have no money.
So I was in Montreal for the weekend, and I couldn't find a Bureau de Change anywhere. The only person I could find was this complete mess of a man looking in trashcans for lunch.
I asked him "can you make change?" and I showed him a fifty dollar bill U.S..
I got back five dollars Canadian.
