Really is an adverb use to describe a certain truth, false interest or a well rounded basic description for don't give a fuck. Can be used in various situations to basically get your point across. Best damn word ever created.
Examples: Thats what she said! Really Johnny? My mom is hot, really? Isn't she just the sweetest bless her little heart! Really? YOUR REALLY STUPID
An utterance of shear amazement at the complete lack of common sense or the complete lack of awareness of ones surroundings. Typically while driving.
(On cell phones)
Andrew: "So yo, homeslice, what you up to?"
Dave: "Just driving to work dealing with my daily dose of fuck nuggets... you?"
Andrew: "Yea, just han.... Really!?? I mean Really!!?? Am I fucking invisible dude!!? Pay attention to your surroundings you douche licking ass pipe! Yo, sorry man, just trying not to get run off the road.

A meme-ish inside joke that you would understand if you watched Andrew Bravener on BlogTV (Dec. 16th,2009). Some dude who called himself Keith pretended to be a moderator and threatened to ban Andrew because he was playing "copyrighted music". AB, annoyed yet mildly amused, responded to the troll with these three simple yet epic words. The phrase is now a Twitter hash tag (#reallykeithreally), and Andrew has made the first "Really, Keith, Really!?" t-shirt.
His fans soon followed suit.
It can also be used in a similar style to FML.
