The fail rating system; All fails are bad but some require their own particular term. The heirarchy is as follows:
1. Fail - bad but survivable
2. Epic Fail - Worst fail most people acheive, should make you at least consider suicide and/or mass killings to hide your shame.
3. Jail Fail - Name says it all
4. Death Fail - only useable for darwin award nominees
Only dumb MFs don't properly apply the hierarchy of fail.

A Ben Fail, far worse then any type of "fail" or "epic fail", would be called a Ben Fail. Ben Fails occur rarely or commonly depending on how idiotic the person is. The name "Ben Fail" originates from a disturbed teenager that lives in Northern Michigan that struggles with... many things. So now he has recognized his failure and now thinks the world shall know.

A failure ( or fail ) so disastrous that it achieves the total reverse of what was originally planned because the effect of an action has become so dissociated from the intent behind it.
A reference to the Clint Eastwood movie 'The Eiger Sanction', in which Clint, an assassin, is so convinced his target is a member of a teem of mountaineers climbing the Eiger - he just doesn't know which one. So he murders all of them just to make bloody sure - then finds out he was wrong all along, like a moron, because it's been obvious as hell all along who the real bad guy is.
