Sharp elbows

In its most basic sense, it means pushing through a crowd.
But it is also used for a more metaphorical sense. E.g. for people who don't mind pushing other people out of the way or stepping on others to get what they want.
In other words, it means to be aggressive or ambitious, usually at the expense of others.

Young employees have sharp elbows these days. They dream more about management positions and high salaries than their older colleagues.

Synonymous to "tennis elbow," AP elbow is the extreme pain one feels in the elbow after the writing section of an AP exam. This is typically the result of writing three essays in two hours or four essays in 100 minutes.

Assholes and elbows

'assholes and elbows' refers to working hard at a task. It originated in farm work. If a group of field hands is busy and bent over picking crops, then a supervisor looking out over the group would see nothing but "assholes and elbows".
A derivative term with the same origin and meaning is head down, butt up.

Row boss to field hands: Alright that's enough chit-chat and coffee break! It's time to get back to work and I don't want to see nothing but assholes and elbows until this field is picked clean.

During the act of a high five, if one is having trouble, all he need do is "Watch the Elbow." If two people are going for a high five and both look at the others elbow, the chances of a "miss" decreses nearly 100%. It will work with less precision if only one person watches the elbow.
