In many ways like a chain fail, but which tends to propagate down a line like a domino effect. That is, if when A fails then B fails, and when B fails then C fails, then if A fails, C fails.
Because Adam didn't fill the car up with gas, his girlfriend, Becka couldn't get to work on time. Because Becka couldn't get to work, she could not add the sales figures to Craig's presentation. Because Craig did not have the sales figures, the client fired Craig. Craig, in turn, reported Becka's habitual tardiness to management, which got her fired. Because she was fired due to Adam's lack of forethought, she subsequently dumped him.
Since when Adam failed, Becka failed; and when Becka failed, Craig failed, Adam created a transitive fail.
Astronomical fail. Something going so wrong as to require an extra adjective (astronomical) to accurately describe how terribly awful something went. Can also be used to describe a person who fails at something or just life in general.
"Dude, that part last night was an astro fail."
"You clearly can't dance. Astro fail."
"You are such an astro fail sometimes."
A fail pwn is best described by using an example. Say a friend of yours requests back-up in any video-game, and you tell them you can't play because you are too tired; this would be a friend fail. If on top of friend failing, your friend gets slaughtered in whatever game they played and lets you know the next day about it, the appropriate response from the absent friend would be, "fail pwn," since by their friend fail, the other was pwned.
Jonny: Wow, you left me to die last night on counter-strike. We got completely slaughtered. What a fail.....
Sam: Yeah, I know. It was definitely a fail pwn.
When a person failure is so far beyond the limitations of the word fail.
Person A: I just met a guy at a bar who told me he loved me, I was so happy that I slept with him.
Person B: Really? You have Failed^2 in life.
Adult children that briefly left home that couldn't make it in the real world and moved back into their parents home (nest)
Did you hear that the Cadman boy moved back home after he couldn't get a job after eight years of college getting a doctorate in Theater Arts? I feel sorry for them about their failed fledgling.

A "Fail" that is a bigger fail than a basic fail but not quite an "Epic Fail." A Fail that is rapidly barrelling towards Epic Fail proportion.
This drive-through is a turbo fail. The parking lot exit uses the drive through lane, and you can't see cars coming from the drive through.
