To be a right wing snowflake who is totally shook (shaken) at the sight of or mention of milkshake.

Also to have been struck by a milkshake for being a right wing or crypto fascist dickhead.

“Fascist bed wetters be like I feared for my life when they saw what beverage I bought from McDonald’s.”

The feeling any right wing extremist public figure experiences when going outside of their habitat. It is the fear of being hit by a milkshake (a BK one in Scottland) as a form of public protest against their beliefs or actions.

A: "Hey, Sargon of Akkad has another public speech today in Swindon"
B:"WOW, he's still going? I thought he'd be too milkshook to even leave his house by now"
