Wth is a slang way of saying what the heck or what the hell.
If you add on the "a" after the "t" and before the "h" it would mean what the ACTUAL heck or what the ACTUAL hell.

"I HEARD A LOUD SCREAM IN THE ATTIC!" shouted Amy.
"Wtah, are you sure or was it just your imagination?" whispered Sophie.
"Wth? How would I just hear a scream in my head?" muttered Amy
