A variation of 'what the fuck', an expression of disbelief.
WTFO, however, is used when the disbelief is caused by a betrayal - when you betray someone you 'fuck them over'.

Bill: I told your g/f you ditched her last night to go bar-hopping.

Dave: What the fuck, over? You know I was at work covering for you!
