To vomit from drinking too much. The mouth is like coins coming out, and like a one-armed bandit, the arm is up from holding one's hair out of the way, or from a toilet or whatever you are handing onto. Most don't even notice the irony, of all what one wins when one is excessive and greedy.

Allowing something to build and then dumping it on someone all at once. Often this is a workplace discipline issue where many minor or even not minor events are allowed to build without notice of violation and then once sufficiently large can be used against an employee all at once to justify a large penalty or termination. Can also be used in an emotional sense in the instance of a lover or family member who at one time recites all the real and perceived misdeeds against them despite having never voiced displeasure previously.

Jack was never told that sitting at his desk one minute late daily was a problem until 6 months into his job his manager jackpotted him with a cumulative 130 minute tardy violation for which he was terminated.
