Pilot Mode

When software is installed or developed directly into a production system without clearly defined requirements or any form of testing.
This software usually remains dormant or is lightly used until such point as the warranty period expires and full usage begins whereby all the defects and failings become apparent.

T=Testing

PM. "We are under pressure to deliver this on time therefore we will launch it in Pilot Mode."
<30 days later>
TL. "It's in production now and people are unhappy as it doesn't work."
PM. "It's not my problem as the warranty period has expired."
TL. "We can't even begin to fix it as there is no Dev or Test environments which contain this software, nor is there any documentation!"
